During open enrollment, you can change your 2016 enrollment choices throughout the month of October. To enroll in any 2016 benefits, you must complete each step in enrollment process until you see your 2016 benefit statement. Most benefit enrollments are irrevocable during the plan year unless you experience a qualified midyear change event and the request is consistent with the change.

To make midyear changes to your current 2015 benefits, download the midyear change form and submit it to your agency payroll or university benefits office..
PEBB is mailing all members of our self-insured plans - PEBB Statewide and Providence Choice - SSNs of their covered dependents. The Affordable Care Act (ACA) requires that we ask for and provide this information to the IRS. The purpose is to match up provisions of the ACA with employer-provided health care benefits. The mailing gives instructions on providing the information through a secure online system.

If you have received the mailing, click here to enter Social Security numbers by request.
